Description

• Assess patients in either the inpatient or outpatient settings, creating tailored treatment plans that cater to individual needs
• Deliver physical therapy interventions aimed at enhancing mobility, strength, stability, and overall functionality
• Collaborate diligently with nurses, physicians, case managers, and other rehabilitation professionals to maximize recovery outcomes
• Guide patients and their families regarding therapeutic objectives, discharge plans, and home exercise regimens
• Maintain meticulous documentation of evaluations, treatments, and patient progress in line with regulations and facility standards
• Engage in interdisciplinary care rounds and conferences to ensure cohesive patient care strategies

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

• State licensure as a Physical Therapist
• Preferred CPR certification
• Additional specialized certifications (e.g., neurological, geriatric) would be advantageous but not mandatory

Skills

• Prior experience is highly desirable
• Competence in treating patients with complex medical and mobility challenges
• Excellent communication and teamwork abilities
