This full-time opportunity offers a weekday schedule and a $5,000 SIGN-ON BONUS. Position located in Ozaukee County. Case Manager Job Responsibilities Manages an assigned caseload of program members in collaboration with an RN Case Manager Completes comprehensive member assessments, member centered care plans, and functional screens Provides ongoing care coordination and quality assurance monitoring of services Maintains required documentation in adherence to established standards. Case Manager Job Requirements Education: Bachelor’s Degree in a Human Services or closely related field required. A minimum of three (3) years’ experience working with seniors or adults with physical and/or intellectual disabilities may be substituted for a non-human services degree. Experience: One year of experience with older adults or persons with physical and or intellectual disabilities in a social service capacity preferred. Certification: Eligibility for long-term care functional screener certification is required. Travel: Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage.
