Occupational Therapist (PRN)

Location: Yorktown Manor – Yorktown, IN

Schedule: PRN | Flexible Hours | Approximately 10 Hours Per Week

Yorktown Manor is seeking a dedicated Occupational Therapist (OTR/L) to join our rehabilitation team. In this PRN role, you will provide high-quality occupational therapy services to help residents improve their independence, mobility, and ability to perform daily activities.

Position Summary

The Occupational Therapist will implement individualized treatment plans and provide therapy services based on residents’ needs. This position works closely with the interdisciplinary rehabilitation team to promote positive clinical outcomes and enhance residents’ quality of life.

Key Responsibilities

• Follow and implement individualized treatment plans

• Collaborate with rehabilitation team members to ensure treatment plans meet resident needs

• Instruct residents, caregivers, and families on therapy goals, techniques, and exercises

• Assess residents and develop treatment plans with clearly defined short- and long-term goals

• Monitor resident progress and modify treatment approaches as appropriate

• Maintain accurate and timely clinical documentation

Qualifications

• Current Occupational Therapist (OTR/L) license

• Strong communication and collaboration skills

• Ability to work independently and as part of an interdisciplinary team

• Previous experience in a skilled nursing or long-term care setting preferred
