About this Position Bilingual Behavioral Health Community Guide FLSA Status : Exempt Job Status: Full Time Work Model: Hybrid Work Schedule: Monday–Friday with occasional evenings and/or weekends Vehicle Required: Yes Amount of Travel Required: 25–50% within a 30 mi community service area Reports To: Care Team Manager Location: San Antonio, TX – Bexar County Reema Health seeks an experienced, self‑motivated Community Guide who has worked with individuals experiencing serious mental illness and substance use disorder. The role involves collaborating with a multidisciplinary care team to plan and coordinate services for members, providing holistic support, and ensuring seamless integration of care within a hybrid, technology‑enabled workflow. Responsibilities Engage and enroll members from diverse ethnic backgrounds, focusing on severe and persistent mental illness, substance use disorders, and complex medical conditions. Manage a caseload of 60–100 members, providing cold outreach, ongoing follow‑up, and support throughout the care plan lifecycle. Conduct face‑to‑face visits, phone, text, and email communications to build trust and maintain connection to Reema’s clinical services. Assess member needs, including social determinants of health, and collaborate with clinicians to connect members to behavioral health, primary care, and community resources. Coordinate referrals, appointment scheduling, and transportation when necessary. Document all interactions and care plan updates in the Reema App and EHR, ensuring timely, accurate, and HIPAA‑compliant record‑keeping. Represent Reema positively, maintaining professional boundaries, confidentiality, and promoting a culture of dignity and respect for all members. Adapt to evolving startup changes, share insights with the multidisciplinary team, and support the team during periods of increased workload. Qualifications & Position Requirements Bachelor’s degree in Social Work, Human Services, Psychology, Health Sciences, or related field (or substantial equivalent experience). At least 3 years of direct experience working with the target population. Fluency in Spanish. Strong crisis planning, case management, boundary setting, and personal safety skills. Proficiencies in critical thinking, time management, multitasking, and technology (smartphone, tablet, Google Workspace, mobile apps). Valid driver’s license, reliable transportation, and satisfactory driving record. Completion of a pre‑employment drug screening, background check, and compliance with HIPAA regulations. Physical ability to lift and carry up to 25 pounds occasionally.
