Your role involves admitting patients, closely monitoring patient progress and making necessary treatment adjustments, and engaging in meaningful discussions with patients and their families about their care. Additionally, you'll facilitate seamless communication with the patient's primary care team and provide essential medical records to support their ongoing treatment.
