About the Position

Job Title: Hospice Case Manager Profession: Registered Nurse Specialty: Hospice Duration: 13 Weeks Shift: Day Hours per Shift: 8 Experience: Minimum years of experience in hospice care within a home health setting required. License: Active nursing license required. Certifications: BLS, ACLS, and PALS certifications required. Must-Have: Previous experience in hospice care is essential. Description: Home health hospice case manager position available. Experience in the home health hospice setting is required. Local candidates are welcome to apply. Mileage reimbursement will be provided at the IRS rate. No call responsibilities associated with this position. Patient case management duties to include visit management. Required to see a minimum of 5 patient points per day. Various visit types will be based on patient needs and case specifics. The position involves initial evaluations, follow-ups, and recertifications. Visit types will have specific point weights assigned. Supervisory references are required for the application process. Qualifications include required certifications with current licensing. Charting experience with required systems is necessary. All applicants must comply with submitting documentation and references.
