This role reports to the Director of Health & Wellness.What You'll DoProvide direct resident care services and follow each resident's individualized plan of careAdminister medications following standards of practice, state regulations, and required documentationPerform resident assessments as required by state standards and facility policyObserve residents for changes in condition and report immediately to the supervisor or Department DirectorDocument observations, communications, and daily care by the end of each shiftComplete and maintain individualized care plans; attend interdisciplinary care meetingsCheck and record vital signs, weights, and blood sugars as assignedAssist with supervision, coaching, and training of direct care staffSupport fall prevention and a restraint-free environmentWhat You'll NeedHigh school diploma or equivalent (GED) requiredLicensed Practical Nurse license in applicable state requiredCurrent CPR certification requiredSuccessfully complete anAHCA criminal background check.
