WCS Healthcare Partners has an immediate opening for an RN Case Manager with a large social services agency serving individuals in Suffolk County, NY .
Responsibilities
- Assess individuals' medical and healthcare needs.
- Build strong, collaborative relationships with individuals, families, support staff, and treatment teams.
- Develop nursing care plans and ensure support staff follow established plans of care.
- Coordinate and schedule medical appointments in a timely manner.
- Educate individuals and support staff on nursing care plans, medications, treatments, and health needs.
- Monitor and document individuals' health status, follow-up care, and medical needs.
- Communicate and implement prescribed medications and treatment plans with support staff.
- Monitor health records, including seizure charts, bowel movement tracking, vital signs, and other required documentation.
- Ensure medications are stored appropriately, expired medications are disposed of properly, and medication areas remain clean and organized.
- Attend staff meetings, trainings, and other required meetings.
- Communicate important medical updates and concerns to the appropriate members of the care team.
Requirements
- Current New York State Registered Professional Nurse (RN) license .
- Minimum of 1 year of nursing experience working with individuals with intellectual/developmental disabilities (I/DD) or prior OPWDD experience required .

Locations are approximately 5 minutes apart :
- 4 Winter Green Drive, Coram, NY
- 462 North Bicycle Path, Port Jefferson Station, NY
Schedule
Monday–Friday | 8-hour shifts
- 8:00 AM – 4:00 PM or
- 9:00 AM – 5:00 PM
