LVN/LPN In Home Health

Join our team as an LVN/LPN in Home Health, where you'll provide essential care to patients in their homes.

What You'll Be Doing

Conduct home visits for patient care
Document patient progress and care plans
Attend periodic office meetings
Manage a caseload of approximately 58 patients
Travel between patient homes, up to 75 miles from the home clinic
Utilize Homecare Homebase (HCHB) for charting and documentation

What We Need To See

Active LPN or LVN license
CPR and BLS certification
At least 3 years of recent home health experience
Proficiency in Homecare Homebase (HCHB) within the past six months
Willingness to travel up to 75 miles
