As a Nurse Practitioner, you will play a pivotal role in our comprehensive preventative care program, incorporating Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and Collaborative Care Management (CoCM) to minimize hospitalizations and elevate patient care standards.
