TCOOMMI Adult Rural Intensive Case Manager (Kendall and Medina Counties) HILL COUNTRY MHDD CENTERS IS OFFERING A $2,000 SIGN-ON BONUS TO EMPLOYEES Help us help others: Primary Responsibilities Include: The Texas Correctional Office on Offenders with Medical or Mental Impairments (TCOOMMI) Rural Parole/Probation Intensive Case Manager (RICM) is responsible for providing person-centered intensive case management services to adults diagnosed with severe and persistent mental health disorders who are currently on the Special Needs Offender Parole caseload and the Mental Health Initiative Probation Caseload in Kendall & Medina Counties. The position reports to the TCOOMMI Program Director and requires development and maintenance of effective, professional working relationships with Comal County Adult Parole & Probation Officers and other community providers. This is a Monday through Friday, non-exempt, field-based position with flexible hours. The TCOOMMI RICM regularly meets with individuals in their homes, community, parole/probation, court, and Hill Country MHDD clinics to provide case management, skills training, and psychosocial rehabilitative services for a caseload of 12-15 individuals. Staff expectation is to provide 3.5 billable service hours per individual on the TCOOMMI caseload every month. Services are intended to stabilize mental health symptoms, reduce the risk of recidivism, increase the individual's awareness of and participation with community and natural supports, develop self-advocacy, and assist with their ability to participate in independent behavioral health services. A high level of detailed electronic documentation is required, and adherence to company and state guidelines is expected. The person hired in this position should be knowledgeable about the Texas Department of Criminal Justice, TCOOMMI Program Guidelines and Processes, PGP-01.02. The person in this position must possess a passion and aptitude for working with individuals who have chronic and severe mental health needs and are involved in the criminal justice system. Knowledge of psychotherapeutic medications, intensive case management functions, substance use treatment, crisis services, community resources, and evidence-based practices in the mental health and criminal justice systems is required. This position necessitates innovative thinking, patience, resiliency, skills in advocacy, service coordination, mental health assessment and evaluation, collaborative person-centered recovery planning, and dual-diagnosis treatment. The person in this position will effectively work independently and on a team, prioritize needs during crisis situations, engage in regular meetings and training, balance self-care, and perform other job duties as assigned with a positive attitude. Promote and maintain a trauma-informed, person-centered environment for all clientele and staff. Ensure services provided are in accordance with the TCOOMMI PGP for RICMS. Regularly communicate with the Program Director with feedback, concerns, and solutions to meet expectations. Initiate contact with individuals served and/or Legal Authorized Representative (LAR) to assess needs, provide collaborative services, person-centered recovery planning, facilitate referrals and linkages to appropriate resources, monitor service effectiveness, provide advocacy, facilitate crisis intervention or prevention services. Provide services in a timely manner. The services are in accordance with Health and Human Services and TCOOMMI contract/guidelines. Initiate and reciprocate coordination and collaboration with members of the Recovery Team and/or other Intensive Recovery Teams, Program Director, Center Directors, peer specialists, and other Hill Country employees/departments. Initiate and reciprocate coordination and collaboration with community members, as appropriate, to better provide quality services, including support delivery. Coordinate intake and assessment of individuals referred to the TCOOMMI program. Ensure that clinical documents and tools necessary for state contracts are completed electronically in a timely manner to ensure services are clinically appropriate and documented. This includes completion of, but is not limited to, uniform assessment Adult Needs and Strengths Assessment (ANSA), Person-Centered Recovery Plans (PCRP), Demographics, Diagnosis, and Financial Assessments. Ensure that all individuals have a current PCRP that identifies intensive case management as an intervention. Identify the psychosocial needs of the individual and facilitate their attendance at related services. Identify and coordinate the individual's access to all available therapeutic and behavioral health services, including inter- and intra-agency resources. Serve as an advocate for the individual. Provide individual service in the areas of activities of daily living, coping skills, problem-solving, stress and anger management, social and recreational skills, psychiatric symptom management, effective communication, and psychosocial functioning recovery. Ensure all service activities are clearly documented in the electronic health record system, and documentation indicates interventions performed, progress made, and the relation of services to recovery plan goals. Provide collaborative documentation at least 95% of the time. Document all services to include kept appointments, no-shows, cancellations by individuals, and self. Meet the agencies' current productivity standard for the position. Provide purposeful, thoughtful transitional and/or discharge services with approval and consultation of the Program Director and/or with otherwise assigned clinical staff, as appropriate. Develop and maintain strong, professional, and effective collaborative working relationships with parole, probation, and other community agencies. Develop and maintain strong, professional, and effective collaborative working relationships with treatment team members. Participate in regular clinical supervision with the Program Director and/or designee. Participate in professional training, center meetings, and staff development training as required. Adhere to all Hill Country MHDD policies and procedures. Protect client confidentiality by being knowledgeable of and adhering to HIPAA regulations. Maintain ANSA Certification. Travel and driving company or personal vehicle as required for service delivery and professional development. Contribute to a positive, collaborative work environment. Cooperative, cordial. And honest interactions with all employees are observed. Report to work in a dependable manner, regularly on time. Request approval for scheduled leave in advance. Does not show evidence of excessive use of unscheduled leave or tardiness. Participate in coverage for team members as assigned. Maintain computer and software competency for specific program areas. Must become certified in PMAB, First Aid, and CPR within three months of employment and maintain certification without interruption thereafter. Maintain certification in CPR AED. Responsible for staying current in mandated training and receiving additional training as appropriate. The job description may be changed to reflect job updates and requirements. Complete all incident reports accurately and in a timely manner according to TCOOMMI PGP. Utilize appropriate program reporting tools to communicate with the Program Director regarding admissions, discharges, and incident reports. You'll definitely need: Bachelor's degree in Social Sciences with the ability to be credentialed as a Qualified Mental Health Professional Preferred: Master's degree in the Social Sciences field from an accredited university.
