You’ll also oversee visit frequency and coordinate with physicians, office staff, and other disciplines involved in the patient’s care.Your responsibilities will include:Performing patient assessments and skilled nursing visitsCompleting accurate OASIS assessments and clinical documentationAssessing pain and implementing appropriate pain-management techniquesReviewing patient history, diagnostic information, and lab results and reporting abnormalities as appropriateMaintaining an accurate medication profileCoordinating care and communicating patient needs with physicians, office staff, and other members of the care teamOverseeing visit frequency throughout the patient’s episode of careParticipating in weekly case conferences and monthly case manager meetingsYou’ll have an important role in making sure the many pieces of a patient’s care come together effectively while providing compassionate, respectful care in the home setting.RN Case Manager QualificationsCurrent, valid California RN licenseStrong clinical assessment, communication, organization, and documentation skillsAbility to coordinate effectively with patients, families, physicians, and interdisciplinary team membersCommitment to patient-centered care and respect for patient dignityAbility to work independently while maintaining strong communication with the care teamValid driver’s licenseReliable personal vehicleCurrent automobile insuranceAbility to travel to patients’ homes throughout the assigned service areaHome health experience is beneficial.
