Job Title: Care Coordinator Profession: RN (Registered Nurse) Specialty: Case Management Duration: 13 weeks Shift: 8H Days Hours per Shift: 8 hours, 08:30-17:00, 2 weekends a month Experience: Minimum 2 years License: CA RN License Certifications: BLS Certification, LA City Fire & Safety Must-Have: Prior experience in Care Coordination Description: MUST HAVE PRIOR EXPERIENCE OR APPLICATION WILL NOT BE CONSIDERED. Candidates must be certified in Basic Life Support (BLS). A valid California Registered Nurse license is required. The position involves providing care coordination in an acute care setting. The charting system utilized is Cerner. Team members will be scheduled for 40 hours per week. Cancellation policy allows for up to 4 shifts to be canceled during the assignment. Solid experience in Care Coordination is essential for this role.
