Overview Illnois, United States | Posted on 03/02/2024 Under the direction of the CEO, the Licensed Clinical Social Worker (LCSW) will provide direct services to senior adults and caregivers from a variety of referral sources. This position connects patients and community members to program offerings and community resources and may also serve as a representative. The LCSW will also work in conjunction with the patient’s family, caregiver, nurse, and clinician to provide optimal support services. This position will help the patient gain the strength to carry on with daily life-like skills, improve quality of life, and help them remain independent. Responsibilities Provide mental or clinical social work services both telephonic and in the home of clients. Carry out comprehensive social work assessments by phone or in-person via home visits to develop a plan of care for the client. Work directly with patients and their families to address unmet needs and provide the necessary social work services to help patients and support staff achieve their care goals. Maintain relationships with referring partners through consistent and timely communication. Promote the STARS program and the STARS CRDC Resource Centers program offerings by helping clients connect with programs that address their needs. Evaluate patients (existing, new, and prospective) based on their needs, limitations, and desires. Address concerns and goals while maintaining constant communication with the patient. Build rapport with patients and their families. Coordinate care with PT and OT. Respond to crisis situations that affect vulnerable or mentally unstable seniors. Advocate and raise awareness on behalf of clients and the needs of the community. Required Qualifications Master's degree in social work, psychology, sociology, healthcare, or another related field. Valid and current LCSW (Illinois). 2-5 years of professional social work experience. Experience in case management. Experience working with geriatric patients. Master’s in Social Work (preferred but listed here for completeness). Preferred Qualifications Home Care Hospice experience. Proficient in Microsoft Office Suite. Confidentiality. Assessment, counseling. Excellent listening and communication skills. Able to work nights and weekends. Must have transportation available to travel to meet with clients. Self‑directed and able to work without supervision. Extensive background in rehab facilities, hospital depression, trauma. Positions & Compensation Part‑time and full‑time positions available. Paid for visits upfront twice monthly—no waiting on insurance for your payment. 90‑Day incentive bonus ranging from $300 to $600 and a first‑year incentive for maintaining volumes. Both in‑person and telehealth visits with clients—must be willing to conduct some visits in‑person. Territory of your choice in Illinois or Nevada. 1099 contractor, full‑time or part‑time—flexible schedule of your choice with all positions (evenings/weekends available).
