Role Summary The Discharge Coordinator plans, manages and coordinates discharge plans for acutely hospitalized patients, working closely with the multidisciplinary care team to ensure smooth transitions across the continuum of care. The coordinator is knowledgeable of community resources and applicable healthcare benefits and demonstrates excellent written and verbal communication skills in a fast‑paced environment. Responsibilities Develop and implement discharge plans in collaboration with physicians, nurses, social workers and other relevant team members. Coordinate necessary community resources and services to support patients’ post‑hospital care. Maintain accurate documentation and ensure compliance with regulatory guidelines and institutional policies. Required Qualifications Previous experience in acute care, healthcare or a related field. Preferred Qualifications One year or more of experience in hospital discharge planning. Knowledge of Medicare/CMS guidelines, TJC standards, and NCQA standards. CA RN or LVN license. Physical Requirements Dexterity, coordination, and visual/auditory acuity to perform all job functions. Ability to sit or walk for extended periods. Capacity to work effectively in a busy, distracting environment.
