LPN, Primary Care Internists (Per Diem)

Purpose of Position Under supervision of the Clinician and Practice Manager and in collaboration with other team members, provides direct and indirect patient care in accordance with hospital policy and procedure. Description Under the direct supervision of the Physician and Practice Manager, assists in providing clinical care to office patients. Greets and escorts patients to exam rooms. Obtains relevant information and takes vital signs as indicated (height, weight, blood pressure, heart rate, respiratory rate, oxygen saturation). Inputs pertinent clinical information into the electronic health record timely and with accuracy and performs a complete medication reconciliation. Triage incoming calls, messages, and In-Basket tasks; prioritize urgent matters and as needed. Documents all calls, encounters, and actions taken into the patient’s medical record timely and completely according to established policy and professional standards. Participates in identification of the aspects of care for the patient, indicators used to monitor the quality and appropriateness of care, and the evaluation of that care. Adheres to safety, blood/body fluid policies and procedures. Assists clinicians with clinical procedures and/or additional tests specific to the department. May advise patient of preparation required for exam or additional testing and keeps patients informed of visit status, unforeseen delays and other relevant information pertaining to the patient’s visit. Verifies and implements clinician’s orders for testing and/or procedures (EKG’s, Holter monitors, etc.). Under the supervision of a physician, administers routine injections when delegated, and documents in real time. Prepare and maintain exam rooms for patient arrivals. Clean and/or maintain inventory of supplies (medical included) and equipment throughout the practice. Notify supervisor to order clinical supplies, clinical equipment, pharmaceuticals and assists in resolving patient concerns by acting as an advocate, as appropriate. Participates in practice and department meetings, trainings, etc., to promote continuous improvement and quality patient care. Takes initiative to learn, understand, and stay current with community resources and the greater healthcare system while continuously participating in self-assessment of skills. Maintains patient rights and abides by all confidentiality obligations pursuant to Cape Cod Healthcare policies and HIPAA. Provides clerical/office support as necessary (greeting, registering patients, scanning incoming medical records and paperwork, preparing medical records, preparing letters, answering phone calls, taking messages, faxing, etc.). Triages walk-in patients per established protocols. Cross-coverage due to illness, vacation, or unexpected absences for all PCI Physicians. Maintain CEU requirements required by licensing board and provide documentation to supervisor. Maintains communication and collaboration with inter‑professional team and others to facilitate safe transitions and continuity of care delivery. Consistently provides service excellence to all patients, family members, visitors, volunteers and co‑workers. Performs other work‑related tasks and activities as assigned or requested by manager or designee. Qualifications Ability to read, write and communicate in English Graduate of an accredited School of Nursing Current Massachusetts LPN License Basic Life Support certification (BLS) and must maintain certification Strong interpersonal, customer service, time management skills Experience with current office technology and proficient with computer office applications (Electronic Medical/Health Records, MS Office, etc.) A minimum of two (2) years’ experience in a clinical setting preferred Schedule Details Per Diem, Mon-Fri, 7a-5p, 7:30a-5:30p, Occasional weekends and Holidays Organization: Cape Cod Hospital Primary Location: Massachusetts-Hyannis Department: CCH-PCI - Hyannis Annual/Hourly: Hourly Hiring Pay Range: 30.69 - 64.98 #J-18808-Ljbffr Cape Cod Healthcare
