Job Duties as a Transitional Care Coordinator:
- Develop relationships specific to area to assist in maximizing census
- Increase referral rate from area hospitals
- Monitor and evaluate the effectiveness of transitional care interventions, making adjustments as needed to optimize patient outcomes
- Maintains knowledge of current regulations governing Medicare, Medicaid Insurance
- Embrace company's core values and incorporate them into daily job
- Demonstrate respect and compassion in every interaction
- Perform other duties as assigned
- Conduct comprehensive assessments of patients medical, social and support needs to develop individualized transitional care plan
- 80% - 90% of time spent in communities marketing
Qualifications for Transitional Care Coordinator:
- 2 years of relevant experience in Health Care Marketing or Business Development
- Bachelors Degree
- Must be 18 years or older
- Ability to deal tactfully with personnel, residents, family members and the public
- Exceptional people skills
- Excellent verbal, written and interpersonal communication skills
- Ability to work independently
- Must have good time management
- Ability to prepare accurate reports
- Must be proficient in the use of a computer
- Have knowledge of healthcare systems, community resources and discharge planning processes
- Must have empathy, compassion and patient centered approach to support patients in transitions of care
