Contract | 13-Week Assignment | Full-Time | Onsite | Days | Monday–Friday, 8:00 AM – 5:00 PM

Key Responsibilities

• Provide outpatient physical therapy services to an average of 14–16 patients per day
• Evaluate patients and develop individualized treatment plans to address functional deficits and rehabilitation goals
• Deliver evidence-based therapeutic interventions in an outpatient clinical setting
• Document patient progress, treatment plans, and clinical notes in accordance with facility and regulatory standards
• Collaborate with interdisciplinary care team members to ensure coordinated patient care

Required Qualifications

• Active Physical Therapist (PT) license in the state of Wisconsin or eligibility to obtain licensure
• Graduate of an accredited Physical Therapy program
• Current CPR/BLS certification
• Experience working in an outpatient physical therapy setting

Preferred Experience / Skills

• Demonstrated ability to manage a high patient volume in an outpatient environment
• Strong documentation and time management skills
