Therapy – Physical Therapist (PT)

Location: Lakeland, FL | Home Health

Schedule / Job Type: Contract | Days | 5×8 Hours (8:00–17:00) | 1–2 weekends per contract

Key Responsibilities

• Conduct skilled home health visits, with an expected 6–8 visits per day
• Perform comprehensive patient assessments including cardiopulmonary, circulatory, neurological, and orthopedic evaluations
• Complete OASIS documentation and maintain accurate records using electronic systems and portable equipment provided
• Develop and implement Home Exercise Programs and provide patient and family education
• Identify and report signs and symptoms of physical, sexual, and psychological abuse; apply safety and injury prevention practices
• Manage billing documentation and adhere to skilled home care requirements for Medicare, Medicare Advantage, and commercial insurance

Required Qualifications

• Active Florida or Compact Physical Therapist license required at time of submission
• BLS certification required
• Minimum 1 year of physical therapy experience required
• Home health experience required
• Valid driver's license and personal vehicle required for home visits
• Must be COVID-19 vaccinated or have an exemption form on file; flu shot or declination required (mandatory masking applies in either case)
• Candidates must reside more than 100 miles from the facility to be eligible

Preferred Experience / Skills

• OASIS training preferred
• Cerner EMR experience preferred
• Experience treating a diverse patient population including ortho, neuro, cardiac, post-surgical, COPD, CHF, CVA, Parkinson's, and trauma patients
