Job Summary:

Responsibilities:

Provide direct patient care to residents in accordance with their individualized care plans and nursing standards

Assess patient needs, develop and implement care plans, and collaborate with other healthcare professionals to ensure comprehensive care

Administer medications, treatments, and therapies as prescribed by physicians and credentialed practitioners

Monitor patient progress and report any changes or concerns to the healthcare team

Develop and maintain effective relationships with patients, families, and healthcare partners

Participate in quality improvement initiatives and contribute to the development of policies and procedures

Collaborate with other departments, including therapy, social services, and dietary, to ensure seamless care transitions and optimal patient outcomes
