Physical Therapist - Home Health (Dothan, AL)

Schedule: Monday-Friday, 8:00 AM-4:30 PM

Weekly Hour Guarantee: 40 hours

On-Call/Weekend Requirements: Occasional weekend admissions

Mileage Reimbursement: Yes, reimbursed between visits, from first patient to last patient

Coverage Areas: 50 mile radius of Dothan, AL

Required License: Alabama Physical Therapy License

Position Overview

We are seeking a dedicated Physical Therapist to join our Home Health team serving Dothan, AL. This role is ideal for a clinician who values autonomy, one-on-one patient care, and the opportunity to help patients regain mobility and independence in their home environment.

Primary Responsibilities

Assess functional status and determine physical therapy needs; collaborate with the physician to develop the plan of care.
Provide treatment within Alabama PT practice laws, following the plan of care and coordinating with the interdisciplinary team.
Observe and report patient responses and changes in condition to the supervising nurse and/or physician.
Deliver interventions to relieve pain, restore function, and maintain maximum mobility using accepted standards of practice.
Educate patients, families, and care team members on the plan of care, functional limitations, and progress.
Complete visits per the treatment plan, submit progress notes per policy, and maintain accurate, timely documentation.
Complete evaluations and develop PT plans of care within state guidelines.
Communicate evaluation results, goals, and projected frequency of care.
Provide care in diverse home environments regardless of patient background or circumstances.

Required Qualifications

Current Physical Therapy Licensure in Alabama
Valid Driver's License, vehicle insurance, and reliable transportation
CPR Certification (or ability to obtain within 90 days of hire)
