Humana Inc. seeks an Onsite Care Coordination Nurse in Hazel Crest, IL to assess newly admitted members and guide them through discharge planning. The role blends in-person facility work with remote documentation, fostering a seamless care transition from hospital to home. Qualified candidates hold an IL RN license, 3+ years in acute care, and experience with discharge planning or care coordination.
