Job Description
Title: Registered Nurse - Acute Care/Home Health & Hospice

Schedule:
- Shift: 8-hour Days
- Days: Monday through Friday
- Start Time: 8:00 AM
- Team Meeting: Every other Tuesday at 7:00 AM

Assignment Details:
- Length of Assignment: 13 weeks
- Hours per Week: 40

Requirements:
- License: RN
- Certifications: BLS
- Experience: 1+ year in acute care, home health, hospice, or inpatient nursing
- Compliance: Nicotine-free facility

Roles & Responsibilities:
- Provide direct nursing care to patients in acute care, home health, and hospice settings.
- Collaborate with interdisciplinary teams to create and implement care plans.
- Monitor patient health and make necessary adjustments to care plans as needed.
- Educate patients and their families on health management and care procedures.
- Participate in team meetings and coordinate care with other healthcare professionals.

Compliance / Notes:
- On-call requirements: 1-2 evenings per week and weekend on-call from Friday evening through Monday morning at 8:00 AM.
- Weekend shifts on a rotation basis with the option to trade with other nurses.
- Holiday work may be required based on unit needs.
- Orientation details will be provided upon selection.
