Hospice Medical Social Worker - LMSW/LCSW

Tender Care Home Health & Hospice is hiring a Hospice Medical Social Worker - LMSW/LCSW to support patients and families throughout El Paso County.

This is a meaningful role for a social worker who understands the emotional, practical, and family challenges that come with hospice care. Our patients and families need someone who can listen, guide, advocate, connect them with resources, and help them feel supported during a difficult time.

In this role, you will work closely with our hospice interdisciplinary team, participate in IDG meetings, provide psychosocial support, assist with care planning and community resources, and help lead our Hospice Volunteer Program.

What You'll Do

As a Hospice Medical Social Worker, you will support patients and families across homes, facilities, hospitals, and other care settings throughout El Paso County.

Key responsibilities include:

• Conduct psychosocial assessments for hospice patients and families.
• Assist the physician, hospice team, and IDG/IDT members in understanding the social, emotional, cultural, financial, and family dynamics affecting patient care.
• Participate in the development and coordination of the hospice plan of care.
• Provide emotional support, crisis intervention, family support, and resource guidance.
• Assist patients and families with advance care planning, end-of-life planning, placement resources, funeral planning resources, Medicaid/Medicare resources, and community support programs.
• Participate in IDG/IDT meetings and contribute to interdisciplinary care planning.
• Serve as a liaison between patients, families, physicians, facilities, hospitals, community agencies, and the hospice care team.
• Support bereavement-related needs and connect families with appropriate grief and community resources.
• Lead, support, and help strengthen the Hospice Volunteer Program.
• Assist with support groups and other hospice-related community or patient support functions.
• Maintain collaborative relationships with clinical and administrative team members.
• Identify and utilize appropriate community resources to support patients and families.
• Maintain timely, accurate, and confidential documentation.
• Participate in discharge planning and education programs as needed.
• Represent Tender Care professionally in the community and with referral partners.

You should be comfortable supporting individuals through grief, crisis, family conflict, serious illness, and end-of-life decision-making. You should also be able to work independently in the field while staying closely connected to the hospice team.

Required Qualifications

• Master's degree in Social Work required.
• Current Texas LMSW license required.
• LCSW candidates are also welcome to apply.
• Minimum of one year of social work experience in a healthcare setting required.
• Hospice experience preferred.
• Candidates with strong healthcare social work experience outside of hospice will also be considered.
• Experience with Medicare programs and community agency resources preferred.
• Bilingual English/Spanish strongly preferred.
• Valid driver's license, reliable transportation, and a good driving record required.
• Ability to travel throughout El Paso County to patient homes, facilities, hospitals, and other care settings.
