This position reports to Physical Therapist of record. ESSENTIAL FUNCTIONSThe Physical Therapy Assistant provides treatments under a plan of care established by the Physical TherapistThe Physical Therapy Assistant participates with all other health care personnel in patient care planningThe Physical Therapy Assistant prepares clinical and progress notes in a timely manner per agency policyThe Physical Therapy Assistant advises and consults with the family and other agency personnelThe Physical Therapy Assistant observes, records, and reports any changes in the patient's conditionThe Physical Therapy Assistant consults with Physical Therapist regarding change in treatmentThe Physical Therapy Assistant educates and instructs the patient, patient's family and agency personnel in use of equipment, home therapy programs and activities of daily livingThe Physical Therapy Assistant assists with the rehabilitation of patients according to the plan of careThe Physical Therapy Assistant participates in case conferences and in-servicesThe Physical Therapy Assistant communicates effectively with all care providers, staff, patient/family, and caregiversThe Physical Therapy Assistant ensures quality careThe Physical Therapy Assistant confirms on a weekly basis the scheduling of visits with the Physical Therapist to coordinate necessary visits with other personnelThe Physical Therapy Assist demonstrates on-going progress toward meeting the criteria for professional practiceThe Physical Therapy Assistant understands and Adheres to all Company policies and proceduresThe Physical Therapy Assistant participates in QAPI activities as assignedThe Physical Therapy Assistant acts as an effective team memberOther duties as assignedKNOWLEDGE & EXPERIENCECurrently licensed by the Oklahoma Medical Board as a Physical Therapy Assistant- no disciplinary action takenMinimum of 2 years of Home Health experienceBLS certificationCurrent OK driver's license and car insuranceMedical knowledge, patience, compassion, kindness, critical thinking skills, knowledge of infection control practices, stamina, and physical endurance, observational skills, knowledge of occupational hazards and safety rules, communication skills, leadership, empathyPHYSICAL REQUIREMENTS The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
