Job Description Summary Full‑time (32 hours per week) variable‑shift position at Berger Hospital. Day shift hours are 7 a.m. – 3:30 p.m. and 8 a.m. – 4:30 p.m., evening shifts are 12:30 p.m. – 9 p.m. Minimum Qualifications and Specialized Knowledge Bachelor of Science or Doctor of Pharmacy degree from an accredited institution (Required) Current Ohio pharmacist license (Required) Basic Life Support Certification (current or obtained during orientation, unless >50% of work hours are off‑site) ASHP accredited residency or equivalent pharmacy practice experience Pharmacy Board Certifications or other certifications Advanced‑level degree in a healthcare field Collaborative practice experience Advanced Cardiac Life Support Certification (may be required) Pharmacokinetic drug dosing experience Drug therapy management experience Job Summary This position is responsible for pharmacist tasks such as dispensing and compounding medication, counseling patients, interpreting prescriptions, managing patient drug therapy, and performing drug utilization reviews. Pharmacists work efficiently independently and as part of a team, use critical thinking and problem‑solving skills, and multitask as appropriate. Strong interpersonal skills, including mentorship, clear communication, collaboration, empathy, and the ability to build trust, are required. Pharmacists collaborate with physicians, nurses, and other health‑care practitioners to provide safe, high‑quality, and cost‑effective drug therapy, may administer medications as allowed by state law, and serve as preceptors for students and residents in their area of training. Acute Care / Infusion 50% – Reviews provider orders for correct drug choice, dosage form, dosing schedule, and potential drug‑drug interactions or allergies; contacts providers to clarify orders; dispenses and compounds medications and drug‑therapy devices according to standards and pharmacy policies; ensures proper labeling and integrity. 20% – Manages individuals' drug therapy by initiating, monitoring, and modifying prescriptions in a consulting relationship with a provider; provides drug information to providers and patients. 10% – Counsels individuals regarding drug therapy, recommends devices, assists with non‑prescription drug selection, and provides instruction. 10% – Provides associate and learner development and training, including precepting students and residents; provides educational programs for pharmacy, nursing, and medical staff. 10% – Directs and guides pharmacy technicians and pharmacy support personnel. Ambulatory Care / Population Health 50% – Manages drug therapy by initiating, monitoring, and modifying prescriptions in collaboration with a provider; prepares individualized care plans to ensure optimal outcomes and adherence. 20% – Partners with patients, caregivers, and health‑care professionals to prevent, manage, and/or slow disease progression through advocacy, wellness, triage, referral, education, and self‑management. 10% – Assists with guideline/policy development and implementation and participates in performance improvement initiatives. 10% – Provides associate and learner development and training, including precepting students and residents; provides educational programs for pharmacy, nursing, and medical staff. 10% – Directs and guides pharmacy technicians and pharmacy support personnel. Retail 50% – Reviews provider orders for correct drug choice, dosage form, dosing schedule, and potential drug‑drug interactions or allergies; contacts providers as needed; dispenses and compounds medications and drug‑therapy devices; ensures proper billing procedures. 20% – Counsels individuals regarding drug therapy, recommends devices, and assists with non‑prescription drug selection; provides instruction. 10% – Manages drug therapy by initiating, monitoring, and modifying prescriptions in collaboration with a provider. 10% – Provides associate and learner development and training, including precepting students and residents; provides educational programs for pharmacy, nursing, and medical staff. 10% – Directs and guides pharmacy technicians and pharmacy support personnel. Medication Safety, Stewardship & Utilization 55% – Leads safety, stewardship, and utilization strategies for designated sites in collaboration with system teams; facilitates committee activities; develops, enforces, and maintains policies that promote safe, effective, cost‑efficient medication use; identifies opportunities for proactive improvement using data and best practices. 20% – Provides associate and learner development and training, including precepting students and residents; provides educational programs for pharmacy, nursing, and medical staff. 10% – Directs and guides pharmacy technicians and pharmacy support personnel. 5% – Reviews provider orders in various pharmacy practice settings; ensures proper dispensing and labeling; prepares care plans. 5% – Manages drug therapy in collaboration with a provider in multiple care settings; provides drug information. 5% – Counsels individuals regarding drug therapy and device recommendations in various care settings.
