Ohio Licensed Care Coordinator

To support youth with complex needs and their families, the full-time Ohio Licensed Care Coordinator will engage clients, conduct assessments, and develop individualized care plans while working remotely in the community. Key responsibilities Effectively engage children/youth with significant behavioral health needs and their families for an assigned caseload Conduct comprehensive CANS assessments and facilitate the development of individualized, strength-based Child and Family-Centered Care Plans Facilitate Child and Family Team meetings, ensuring the child and family have a voice in their care and access to necessary resources Required qualifications At least 3 years of experience in community-based services for children and youth, or relevant education and experience combinations Background in family systems, community resources, case management, or related fields Culturally competent with experience managing complex cases involving children and families Completion of high-fidelity wraparound training program recognized by ODM Valid driver's license with an acceptable driving record and insurability through the organization's vehicle insurance carrier
