Job Details:

Schedule: Monday-Friday | 8:00 AM-4:30 PM | 40 Guaranteed Hours/Week | On-Call/Weekend: Yes

Location: Peoria, AZ

Coverage areas: Phoenix, Glendale, Peoria, Goodyear, Litchfield, Litchfield Park, Surprise, Tolleson, Buckeye, Youngtown, Sun City, Sun City West, Anthem

Mileage reimbursed: Yes

Job Type: Contract

Contract Length: 14 Week/s

Pay Range: $34-$36

*Pay offered is based on experience, expertise, credentialing, and education.

Duties:

• Provide subsequent visits for home health patients.

• Provide wound care, catheter changes, compression wraps, trach care, wound vac management, medication management and education.

• Provide disease education, UA, NG tube care, nephrostomy tube care, and trach care.

Requirements:

• License(s): Active LPN license

• Certification(s): BLS/CPR

• Years of experience: 1+

• Skills/Specialties/Technology: Patient care focused
