Position Overview
The Home Health Aide at United Health Services (UHS) Senior Living at Ideal plays an important role in enhancing residents' quality of life by providing compassionate support with daily living activities, mobility, personal care, and comfort in a senior living environment. The Home Health Aide observes and reports changes in condition, supports a safe and welcoming environment, and contributes to exceptional senior care, long-term care, resident care, and healthcare services that positively impact residents, families, and the community.

$15.00 - $20.74 per hour, depending on experience

Job Responsibilities

Essential Functions:

• Assist residents with activities of daily living (ADLs), including bathing, grooming, dressing, toileting, mobility, and transfers to promote comfort, dignity, and independence.
• Support residents with mobility, positioning, ambulation, and restorative care activities according to established care plans and direction from licensed nursing staff.
• Observe residents' physical condition, behavior, and overall well-being and promptly report changes or concerns to the appropriate clinical team member.
• Maintain a clean, safe, and welcoming resident environment by addressing safety concerns and supporting infection prevention practices.
• Assist with meal service, hydration, and nutrition-related needs in accordance with resident care plans and dietary guidelines.
• Document resident care activities, observations, and related information accurately and timely in accordance with organizational and regulatory requirements.
• Collaborate with nursing staff, rehabilitation staff, residents, and families to support quality resident care and a positive living experience.

Scope and Impact:

• Contribute directly to patient safety, comfort, and continuity of care by providing consistent home-based support and reinforcing patient self-care strategies
• Observe and report immediately to the RN (or Supervisor when indicated) any change in patient condition, functional status, safety risks, or home environment concerns that could impact care
• Exercise judgment within defined role parameters by following the plan of care, recognizing escalation triggers, and seeking clinical guidance when needs exceed scope
• Communicate professionally and compassionately with patients and caregivers to support understanding of the care plan while maintaining appropriate boundaries
• Collaborate with the home care team by sharing observations and suggestions that improve the quality, consistency, and patient experience of care delivery
• Manage assigned workload effectively, including travel between patient homes, preparedness with needed supplies, and completion of tasks within scheduled visit timeframes
• Protect patient privacy and dignity during all care activities, recognizing the trust required when providing care in a patient's home

Compliance and Professional:

• Maintain required certifications, a safe driving record, and ongoing competency consistent with role requirements and New York State expectations for home care services
• Maintain a valid driver's license and, when operating a personal vehicle for business purposes, current vehicle registration and automobile insurance
• Perform only those care activities authorized by the plan of care and permitted under New York State guidance for HHA services, following RN and agency direction
• Complete required orientation, training, and competency validations (including infection prevention and privacy practices) within established timeframes
• Complete accurate, timely documentation of care and patient observations according to agency standards and applicable regulations
• Participate in supervision and feedback processes with the Clinical Management Team and assigned RN to support safe practice and continuous improvement
• Report incidents, near misses, and safety concerns promptly and cooperate with follow-up activities that strengthen quality and patient safety
• Safeguard sensitive data by following established security policies and procedures, using systems responsibly, and promptly reporting potential security incidents, risks, or policy violations
• As applicable to your role, support cybersecurity and data protection requirements throughout the supplier lifecycle by coordinating vendor risk assessments, monitoring third-party compliance, and promptly reporting identified supply chain or cybersecurity risks

Position Qualifications

Required:

• High School Diploma or equivalent
• NYSDOH Home Health Aide (HHA) certification
• Driver License

Preferred:

• Up to 1 year of direct experience performing the exact or very similar duties required by the job within the same role or function
• Up to 1 year of customer/patient-facing experience directly interacting and communicating with the public
• Up to 1 year of experience working in the same work environment or setting (hospital, practice, department/division)
• Up to 1 year of healthcare experience working in the healthcare industry in any job function
• Up to 1 year of technical/systems experience using specialized tools, software, or equipment required for the role
