Social Service Supervisor - Stutz Primary Care

Bon Secours Mercy Health - 3685 Stutz Drive - Responsibilities: Responsible for timely assignment and facilitation of admissions, transfers and discharges based on established milestones.; Anticipates and intervenes to assure clinical milestones are met, including rounding with physician to communicate achievement to the physician/consultant.; Participates/facilitates with biopsychosocial needs of patients.; Helps to establish daily patient goals and help to eliminate barriers in meeting those goals.; Maintains working knowledge of proper patient type and consults with nursing and physicians for disposition planning/barriers to discharge as needed.
