Physical Therapist
Apex Home Health, Hospice & Home Care
Role: Licensed Physical Therapist providing in-home, senior living, and virtual care to older adults. Responsibilities Deliver one-on-one physical therapy in private residences and senior communities. Perform evaluations, develop personalized treatment plans, and administer evidence-based therapies. Support patient wellness, mobility, independence, and aging in place. Document care in EMR systems per regulatory standards. Collaborate with interdisciplinary teams including PTs, OTs, SLPs, and Fitness Specialists. Educate patients, caregivers, and staff on safety and functional living. Utilize telehealth and virtual care to enhance accessibility and continuity. Qualifications & Skills Valid Physical Therapy license or eligibility in practice state(s). Degree from accredited PT program. Proficiency in computer skills and EMR documentation. Current CPR certification. Key Attributes: Autonomy, flexibility, strong patient relationship skills, and ability to meet physical demands of community-based care, including lifting, repositioning, and driving. #J-18808-Ljbffr Apex Home Health, Hospice & Home Care
