Hospice Chaplain - Contra Costa, Alameda, San Mateo, San Francisco Counties

Chaplain Job Summary: Primary function is to provide spiritual care in the home to terminally ill patients and their families. To provide an assessment of the patient’s and family’s spiritual needs and to provide counseling to meet those needs. Makes reasonable efforts to facilitate visits by local clergy, pastoral counselors or other individuals who can support the patient’s spiritual needs. Participates as a core member of the Interdisciplinary Group/Team (IDG/IDT). Job Qualifications: Education: Master of Divinity preferred. Licensure: Not Applicable. Experience: One year of experience as a Chaplain in a healthcare institution preferred. Skills: Working knowledge of community spiritual and other resources. Good interpersonal skills. Transportation: Reliable transportation. Current auto liability insurance and driver’s license if personal vehicle is used for work. Environmental and Working Conditions: Works in patient homes and other healthcare facilities under various conditions. Occasional exposure to infectious diseases; ability to travel locally; may have some exposure to unpleasant weather. Physical and Mental Effort: Prolonged standing, walking, and sitting required. Requires working under some occasionally stressful conditions to meet deadlines and patient/family needs, must be able to make quick decisions; functions as a facilitator to meet patient/family spiritual needs. #J-18808-Ljbffr Zenith Hospice Care
