Center of Illinois Home Healthcare, Inc. is growing and looking for an experienced Clinical Supervisor RN to join our leadership team.

We are a Medicare-certified, ACHC-accredited home health agency focused on delivering high-quality patient care throughout the Chicagoland area. We take pride in maintaining strong clinical standards, supporting our staff, and building long-term relationships with patients, physicians, and referral partners.

This role is ideal for a nurse leader who thrives in home health, understands Medicare regulations, and enjoys mentoring clinicians while helping drive operational success.

What You’ll Do:
• Supervise and support a multidisciplinary clinical team
• Review OASIS documentation and Plans of Care
• Ensure compliance with Medicare, CMS, and ACHC standards
• Assist with staff education, coaching, and clinical development
• Help oversee patient care coordination and clinical operations
• Work closely with intake, scheduling, QA, and agency leadership
• Participate in case conferences and quality improvement initiatives
• Support clinician accountability, productivity, and documentation standards
• Serve as a clinical resource for field staff and office personnel

What We’re Looking For:
• Active Illinois RN license
• Minimum 2 years of Home Health experience
• Strong OASIS and Medicare knowledge
• Leadership or supervisory experience preferred
• Experience with Axxess EMR preferred
• Strong communication and organizational skills
• Ability to work independently and lead in a fast-paced environment
