Support Counselor - ECM (Lead Care Manager)

Location: San Leandro, CA Job Id: 826 # of Openings: 1 Lead Care Manager (ECM Support Counselor) Join our team as an ECM Support Counselor and take part in transforming the way Medi-Cal members with complex needs receive care and support. As an ECM Support Counselor, you'll serve as the primary point of contact for Medi-Cal members enrolled in Enhanced Care Management (ECM). You will conduct outreach research across healthcare, community-based, and social-service settings throughout the San Francisco Bay Area to identify opportunities for connecting with and enrolling new client-members. You'll develop trusting relationships with members and provide comprehensive, individualized care management that addresses medical, behavioral health, and health-related social needs. Working primarily in the community, you'll coordinate with health care providers, social service agencies, community organizations, and other members of the care team to ensure members receive the right services at the right time. This is a full-time, benefited role with significant community-based work. The position requires flexibility to meet members where they live and receive services, including homes, health care settings, community centers, and other community locations. The ideal candidate is compassionate, organized, culturally responsive, and comfortable building relationships with members in community-based settings. Responsibilities and What You’ll Do Identify and engage with community-based, social-service or healthcare organizations and events throughout the San Francisco Bay Area to generate new client-members Develop and participate in community outreach events to increase awareness of ECM services, foster relationships with prospective clients, and generate new client-member enrollment Serve as the member’s primary point of contact, providing individualized and comprehensive care management Develop person-centered care plans in partnership with members, their families or support systems, and the care team Meet with client-members in their homes and communities to assess needs, provide support, and help remove barriers to care Help client-members navigate health care and social service systems and connect them to appropriate providers and community resources across multiple systems Coordinate referrals and follow up to ensure client-members successfully access recommended servicesli> Maintain regular communication with health plans, providers, community-based organizations, and other members of the interdisciplinary care team Advocate for members and help ensure their preferences, goals, strengths, and needs remain central to their care Maintain accurate and timely documentation in accordance with organizational, Medi-Cal, and health plan requirements Maintain confidentiality and compliance with HIPAA and applicable privacy standards Monitor member progress and update care plans as needs and circumstances change Participate in care team meetings, case consultations, and other activities designed to improve coordination and outcomes Respond appropriately to urgent member needs and help connect members to crisis or emergency resources when necessary Qualifications Ability to obtain and maintain fingerprint and government and Agency required background clearances CPR/First Aid certification Valid California driver’s license with an acceptable driving record, auto insurance showing as an insured driver on the policy and vehicle registration A bachelor’s degree in a social science related field; or an associate degree in a social science related field and have at least six months experience in working with emotionally disturbed children or children who have a serious behavioral problem; or at least two years of experience working with emotionally disturbed children or children who have a serious behavioral problem, and who demonstrate a combination of education, skills, and experience Experience providing case management, care coordination, social services, behavioral health services, health care services, or related support to individuals with complex needs Experience as an outreach coordinator and comfortability with public speaking Knowledge of county and community resources and the ability to connect members to social service programs Strong interpersonal and communication skills Experience working with individuals experiencing complex medical, behavioral health, housing, food insecurity, or other social needs Ability to maintain accurate documentation and manage multiple cases, deadlines, and follow-up activities Ability to work independently while collaborating effectively with an interdisciplinary care team Commitment to diversity, equity, inclusion, cultural humility, and person-centered care Able to interact with co-workers, county representatives, and other treatment team members as part of a cooperative team Possess a high level of integrity, honesty and strong work ethic Experience working with Medi-Cal members or within a Medi-Cal managed care environment preferred Experience working with electronic health records systems Bilingual in English and Spanish is preferred Driving Requirements This position requires the employee to work out in the community throughout the SF Bay Area. Thus, requires possess a valid CA driver’s license, current vehicle registration and valid automobile insurance in their name. Access to vehicle in safe working order is necessary. • 21-$24/hour
• 1.20 per hour for Bilingual Pay (Spanish Only)
