Work Environment

This full-time, onsite position allows the FNP to work in a warm, welcoming environment where you can build meaningful relationships with patients. Key Responsibilities

• Conduct thorough health assessments and evaluations to develop personalized care plans for each patient.
• Perform onsite health risk assessments, identifying and addressing potential health issues early through proactive intervention strategies.
• Educate patients on wellness, nutrition, preventive care, and healthy lifestyle choices to empower them in making informed health decisions.
• Support individuals in making positive behavioral changes to enhance their overall health, including exercise regimens, dietary adjustments, and stress management techniques.
• Manage and monitor chronic conditions such as diabetes, hypertension, and asthma, providing tailored treatment plans and follow-up care.
• Collaborate with the medical staff at the center to ensure cohesive, high-quality care and maintain open communication regarding patient progress.
• Engage actively in community wellness events and health fairs to promote health education and awareness, crafting a stronger connection with the local population.

Compensation and Benefits
