As a nurse practitioner (NP) or physician assistant (PA) at Centerwell Senior Primary Care, you will be part of a collaborative, multidisciplinary team that includes physicians, pharmacists, care coaches, behavioral health specialists, and more—all working together to deliver personalized, high-quality care. You will work on site at our Centerwell The Villages clinic. You will report to our associate medical director.

Required Qualifications

• Master's Degree as a nurse practitioner or completion of a physician assistant program with board certification.
• Active, unrestricted NP or PA license in the state of Florida.
• Active DEA license and Medicare/Medicaid provider numbers or ability to obtain DEA license and/or Medicaid and Medicare numbers.
• One +years of experience in value-based care, geriatric or primary care, managing adults aged 65+ with chronic conditions.
• Proficiency using an EMR.
• If selected for this role, you will be required to be screened for TB.
