Social Worker

St. James Rehabilitation and Healthcare Center is seeking a compassionate and dedicated Social Worker to join our interdisciplinary team. This is an exciting opportunity to make a meaningful difference in the lives of our residents by providing emotional support, advocacy, and care coordination in a skilled nursing environment.

• Conduct resident assessments to identify social, emotional, and psychosocial needs

• Develop, implement, and monitor individualized care plans in collaboration with the healthcare team

• Provide counseling, advocacy, and support to residents and their families

• Assist with discharge planning , ensuring safe and appropriate transitions to home or other care settings

• Collaborate with interdisciplinary team members to promote resident-centered care

• Maintain accurate documentation and records in compliance with state and federal regulations

• Serve as a resource for community services and benefits for residents and families

• Bachelor’s or Master’s degree in Social Work (BSW or MSW) from an accredited program

• Valid New York State Social Work license (LMSW or LCSW preferred)

• Experience in skilled nursing or long-term care preferred

• Strong communication, organizational, and interpersonal skills

• Ability to work collaboratively as part of a multidisciplinary team
