Occupational Therapist (OT) - Outpatient & Home Health

This position is responsible for enhancing, participating, and supporting patient care with a focus on quality and safety. The Occupational Therapist (OT), with a physician referral, is authorized to treat patients in accordance with O.T. Practice Guidelines as issued by the American Occupational Therapy Association. The OT will work in an interdisciplinary manner with other departments to provide quality care. Proficiency and proper utilization of the electronic medical record, including proper documentation, is essential. The position primarily focuses on outpatient and home health settings with occasional inpatient coverage.

Responsibilities

• Provide evaluations, treatment plans, and therapeutic interventions to enhance patient care.
• Collaborate with interdisciplinary teams to ensure comprehensive care delivery.
• Maintain accurate documentation in electronic medical records.
• Adhere to O.T. Practice Guidelines as required.
• Support quality and safety in patient care practices.

Required Experience / Certifications / Licensure

Candidates must possess relevant licensure and certifications as required for practice as an Occupational Therapist. Prior experience in outpatient and home health settings is preferred.
