Registered Nurse (RN) – Home Health

Location: North King County, WA
Job Type: Full-Time
Schedule: Monday–Friday

Service Areas: North Kenmore, Bothell, Woodinville, Kirkland, and Redmond.
Pay Rate: $50–$62.05 per hour, depending on experience

About the Role:

We are seeking a compassionate and dedicated Registered Nurse (RN) to provide skilled nursing care to patients in their homes. This role involves assessing patient needs, developing and managing individualized care plans, coordinating with healthcare professionals, and supporting patients and families throughout their recovery.

Key Responsibilities:

• Provide skilled nursing care and evaluate patients’ ongoing healthcare needs.
• Conduct initial assessments and regular reassessments.
• Develop, implement, and update individualized plans of care.
• Monitor patient conditions, document progress, and report changes to physicians.
• Educate patients and families on nursing care and treatment plans.
• Coordinate care with physicians, therapists, and other healthcare professionals.
• Complete clinical documentation, OASIS assessments, and discharge summaries accurately and promptly.
• Apply appropriate ICD-10 codes and process physician orders.
• Supervise and guide LPNs and CNAs, as needed.
• Participate in patient care conferences and assigned on-call duties.
  

Requirements:

• Current and valid Washington State RN license.
• At least one year of nursing experience preferred.
• Home health experience preferred.
• Valid driver’s license and a reliable, insured vehicle required.
• Ability to travel within the assigned region of King County required.
• Strong clinical assessment, communication, documentation, and patient-care skills.
  

Compensation and Benefits:
