Work Shift 8 Hour (days) (United States of America) Scheduled Weekly Hours 40 Job Description Summary Under supervision and in collaboration with the physician staff, the Advanced Practice Provider (APP) performs physical examinations and treats common episodic and chronic healthcare problems with an emphasis on health maintenance and disease prevention. This position is designed for healthcare professionals, such as nurse practitioners and physician assistants, who possess advanced training and expertise in their respective fields. The primary responsibilities include providing patient care in the clinic setting, performing in‑office procedures, and focusing on patient education. As an APP, you will play a critical role in delivering high‑quality healthcare services and ensuring excellent patient outcomes. Job Requirements, Prerequisites and Essential Functions Pay Range: $87.58 - $102.18 - $119.77 Essential Functions and Responsibilities Assess patient status by obtaining health history through patient/family interviews and chart reviews; assessing presenting illness, risk factors, family history, psychosocial situation, and cultural factors; and performing appropriate physical examination. Order and perform appropriate laboratory diagnostic and other screening tests. Seek other information as needed, including consultation with physicians and other clinicians, for evaluation of illness. Integrate data to determine diagnosis and therapeutic plan, including identification of any health risks. Develop and implement treatment plan by prescribing/dispensing medications and/or injections in compliance with medical practice guidelines and state laws. Instruct patient/family regarding medications and treatments. Educate patients regarding health promotion and illness prevention. Recommend appropriate community resources to meet patient/family needs. Communicate appropriate case management information to other professionals and community agencies. Prepare documentation for EMR including updating patient records by posting examination and test results, diagnosis, medications, and treatment. Participate in peer reviews, chart reviews, staff education, clinical guideline development, and other continuing education and quality assurance activities to demonstrate compliance with standards, regulations, policies, and procedures. Act as consultant/collaborator to clinical team on area of specialty. Comply with patient confidentiality requirements. Promote patient advocacy. Collaborate with physicians in managing acute and long‑term medical needs of patient. Provide monitoring and continuity of care between visits according to treatment plan, including triaging patient calls/e‑mails. Perform all other duties as required or assigned. Assessment Elicit and document a complete health history. Perform and document a complete physical examination. Order appropriate diagnostic tests. Assess patient’s perception of their health status, personal strengths, support systems, and patient problems that include psycho‑social, physical, environmental, economic, and cultural factors. Identify patients and families at risk for specific healthcare problems. Diagnosis and Planning Formulate diagnoses based on health history, physical examination, and diagnostic tests. Formulate and document health care plans with patients and their families, including treatment, health maintenance education, and counseling. Establish, review, and document short and long‑term goals in collaboration with each patient. Plan comprehensive patient care with other health care providers. Implementation Demonstrate ability to provide appropriate care based on needs of a specific individual, including that individual’s age. Institute treatment plans based on assessments and interpretations of diagnostic tests and physical findings. Counsel patients according to their individual motivation and compliance. Provide telephone advice and triage of acute and unscheduled patients. Document all pertinent data and interventions in the patient’s records. Provide refills for routine medications. Institute appropriate action when reviewing abnormal lab data and diagnostic tests. Consult with other providers, as indicated. Evaluation Evaluate the patient’s understanding of and compliance with health care plan by regular follow‑up and reassessment. Re‑evaluate short and long‑term goals with patients and participating health professionals. Education Patient and Family Identify the learning needs of the patients and family members. Assess the patient’s readiness to learn. Provide individualized health education. Develop teaching goals and objectives that meet the patient’s and family health care needs. Staff Function as a clinical resource person. Continuity/Coordination Function as the health care provider for a specified group of patients. Coordinate patient care activities and health care planning among multiple providers, patients, and their families. Communicate and consult with family, staff and unit personnel prior to and during the patient’s hospitalization. May also participate in discharge planning when appropriate. Promote ongoing communication with community agencies to facilitate continuity of patient care. Advocacy Promote the participation of patients and families in planning their care. Assist patients in obtaining information necessary to make an informed decision and supports patients in decision‑making. Inform patients of the Patient Bill of Rights. Provide referrals to community resources when appropriate. Qualifications – Nurse Practitioner Education BSN required; MSN preferred Successful completion of advanced practitioner training and certification as a practitioner (general or specialized) from an approved nurse practitioner program License/Certifications Current Registered Nurse, as defined by the California Board of Registered Nursing and successful completion from a college or university‑based Nurse Practitioner program Active and unrestricted Nurse Practitioner license in state of CA Current Nurse Practitioner Furnishing License and verification of experience to furnish (if applicable) Must hold current national board certification by first day of work and maintain it Must hold an active and transferable DEA license to California as required by the clinical specialty Must be eligible for payer enrollment with CMS Medicare and Medicaid BLS and ACLS Certification (American Heart Association) required Experience 2 years of experience in advanced practice or special preferred 2 years in specialty area highly desired Prior experience in a medical office environment preferred Accommodation Qualified applicants with disabilities may request reasonable accommodation during the application process by contacting Human Resources at View phone number on click.appcast.io or View email address on click.appcast.io.
