Position: Home Health Physical Therapist Assistant (PTA)

Shift: Day 5x8-Hour (08:00 AM - 04:30 PM)

Job Description

We are seeking a dedicated Home Health Physical Therapist Assistant (PTA) to join our team in providing exceptional care to patients in the following areas:

• Area #4: Danville, Martinez, Moraga, Alamo, Clayton, Concord, Lafayette, Orinda, Pleasant Hill, Walnut Creek
• Area #5: Pittsburg, Discovery Bay, Byron, Antioch, Brentwood, Oakley, Bethel Island, Bay Point

Key Responsibilities

• Provide individualized physical therapy to home health patients.
• Follow treatment plans as established by the supervising Physical Therapist.
• Visit up to 6 patients per day while meeting productivity standards.
• Document patient care activities and maintain timely communication regarding patient progress.

Requirements

• Experience: Minimum of 1 year of home health care experience is required.
• Licensure: Must have a valid Physical Therapist Assistant license in California.
• Certifications: BLS certification is required.
• Transportation: Traveler must have reliable transportation for patient visits.

Work Schedule

• Hours: Guaranteed 36 hours weekly.
• Must see 5-6 patients per day to meet productivity standards.
• Weekends: Option to pick up patients on weekends to support productivity.
• No on-call duties required.

Additional Information

• Documentation tools will be provided and must be returned at the end of the assignment.
• Free parking is available on-site.
• Mileage reimbursement will follow the IRS standard.
