** This is a part-time position (20 hours). **

 

SUMMARY

Completes client intake screenings/assessments for access to Pillars Community Health (PCH) programs such as but not limited to mental health, substance use disorder, sexual assault and domestic violence services. Assists with providing administrative and operational support for Access and Referral team.

 

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

1. Answers incoming intake calls. Returns intake messages within 24 hours.
2. Completes intake screenings & assessments for Pillars Community Health programs currently part of centralized intake.
3. Uses assessment outcome scores to determine most appropriate level of care for clients
4. Provides resources for callers that are not able to be served within PCH and assists them in making community connections.
5. Conducts initial benefit check for those who have Medicaid and Managed Medicaid plans.
6. Gathers and inputs data into electronic department records in accordance with department needs.
7. Maintains various logs and electronic client records in accordance with department and PCH policies.
8. Outreaches to clients and maintains all logs and tracking systems according to department needs
9. Collaborates with other intake staff in processing client requests for service.
10. Works in collaboration with other PCH programs and staff
11. Participates in organizational meetings, in-services, monthly department meetings, and weekly staff meetings.
12. Coordinates with other community agencies and organizations such as DCFS, schools, hospitals, and other providers in the community as required.
13. Gains knowledge on the different public and private insurance coverage accepted within PCH.
14. Gains understanding of mandated reporting laws and ability to identify when they apply and how to act on them under guidance of supervisor.
15. May represent Pillars Community Health at external service fairs promoting services
16. May provide support in the community to help outreach and engage individuals to participate in services.

 

Administrative Responsibilities (*responsibilities rotate amongst the existing team of Case Managers, and staff will assist on an as needed basis or as assigned)

 

1. Maintains Excel tracking logs and Carelogic reports in compliance with departmental policies, procedures, and reporting requirements.
2. Assists with the coordination, management, and assignment of both internal and external referrals to ensure timely service delivery and appropriate case distribution.
3. Monitors and manages the department's shared inbox, ensuring timely review, response, and routing of communications.
4. Performs data consolidation, reconciliation, and cross-verification between Excel records and the Electronic Client Record (ECR) system to support accurate and reliable reporting.
5. Provides administrative and operational support for client transitions and discharges, including the maintenance and monitoring of program waitlists.
6. Other duties as assigned.

EDUCATION AND EXPERIENCE

Bachelor's degree in nursing, counseling, psychology, clinical social work, marriage and family therapy or related field preferred. At least six months experience in human services or equivalent combination of training and experience. Customer service experience required. Intermediate computer skills required.

 

LANGUAGE SKILLS

Must be Bilingual (English/Spanish) and be able to communicate well verbally and in writing in both languages.

 

DESIRED SKILLS

Seeking candidate with a positive, caring and compassionate attitude toward mental health and those with mental illness. Must also be able to problem solve and make quick decisions during intake calls.

 

SCHEDULE

Scheduling is based on needs of the department and includes some evening hours and may include Saturday hours, depending on need.

 

This position would have an on-site requirement to work Mondays/Thursdays from 2 pm – 6 pm; the remaining hours would be flexible and within the operational hours as listed below:

 

The Access & Referral operational hours for walk-ins are:

 

M, T, W: 8:30-5:30

Th: 8:30-7pm

F: 8:30-5

 

PAY & BENEFIT INFORMATION

 

$18.50 - $22.50 per hour*
