Clinical Pharmacist

City/State Hampton, VA Work Shift Rotating Overview Sentara Careplex Hospital is hiring a Clinical Pharmacist for our Inpatient Pharmacy team! Location Hampton, VA Status Full Time (36 hrs/wk), nine 8 hour shifts per pay period (every two weeks) Shift Details 8 hour shifts, Rotating Days and Evenings (Occasional Overnight). Central shift weekday times begin at 0730 or 1300. Clinical shifts begin at 0700, 0800, or 0900, depending on coverage. Weekends: 1 full weekend and at least 1 additional weekend day per 6 week schedule. Holiday Rotation: 1 of 3 summer (Memorial, Independence, Labor) and 1 of 3 winter (Thanksgiving, Christmas, New Year's). Hours may vary to meet the needs of the pharmacy, and provide occasional weekend or overnight coverage as needed. Responsibilities Demonstrates the ability to work in areas of pharmacy practice competently and independently Manages patient drug therapy in accordance with applicable clinical practice standards Assures the appropriateness of all provider medication orders in maintaining a high level of patient care and safety Maintains current knowledge of medication use and disease states applicable to their area of practice Serves as the primary resource for drug information and education Provides in-depth patient counseling when required Participates in the training of new employees as assigned Assumes local responsibility and ownership for assigned tasks Consistently follows policies and procedures Continually seeks out additional training opportunities and stays abreast of new and changing information for assigned areas Demonstrates superior customer service, effective communication, decision-making, and organization to ensure efficient job performance and success Serves as a role model in the following areas: professionalism, collaboration, and teamwork.
