Job Description: Scroll down for a complete overview of what this job will require Are you the right candidate for this opportunity Job Description Description: The Physical Therapist (PT) provides evidence-based physical therapy services to patients across the lifespan, focusing on restoring functions, improving mobility, relieving pain, and preventing disability. The PT works with primary care providers, specialists, and interdisciplinary care teams to deliver coordinated, patient-centered care in an outpatient community health setting. The PT is compassionate, adaptable, and committed to serving diverse and medically underserved populations. This is an exempt, part-time opportunity (21 paid hours per week). Clinical CareEvaluate patients and develop individualized treatment plans based on clinical findings, functional goals, and patient needsProvide therapeutic interventions including therapeutic exercise, manual therapy, neuromuscular re-education, gait training, and patient educationMonitor patient progress and modify treatment plans as clinically indicatedMaintain accurate, timely, and compliant clinical documentation Care Coordination & CollaborationCollaborate with primary care providers, specialists, and care teams to ensure integrated and coordinated careProvide patient and family education on treatment plans, home exercise programs, injury prevention, and self-management strategiesParticipate in interdisciplinary case conferences and care planning as needed Quality, Compliance & Patient SafetyEnsure compliance with federal, state, and organizational policies, including documentation and billing standardsParticipate in quality improvement initiatives, audits, and accreditation readiness activitiesMaintain licensure, certifications, and continuing education requirements Program Support & Professional PracticeContribute to the development and growth of rehabilitation servicesSupport best practices in patient-centered, trauma-informed, and culturally responsive careParticipate in staff meetings, trainings, and professional development activities Requirements: REQUIRED QUALIFICATIONS AND COMPETENCIESDoctor of Physical Therapy (DPT) or master's degree in physical therapy from an accredited programCurrent, unrestricted New York State Physical Therapy licenseCPR/BLS certificationStrong clinical, communication, and interpersonal skills PREFERRED QUALIFICATIONS AND COMPETENCIESExperience in an FQHC, community health center, outpatient clinic, or safety-net settingExperience working with diverse, underserved, or high-risk populationsBilingual (Spanish or other languages) preferredExperience with electronic health records (EHR) WORK ENVIRONMENT AND PHYSICAL DEMANDSClinical setting with direct patient contact.Requires standing, walking, bending, and occasional lifting consistent with clinical practice.Ability to communicate, hear, see, and feel without require extended periods of computer use for Work at BMS Family Health Center?A mission-driven environment where your work directly benefits the community.A supportive, collaborative team that values your expertise and well-being.Professional growth opportunities within a respected FQHC.A culture of compassion, inclusivity, and respect for all.Competitive Benefits: 20 Days PTO per year.
