Nurse Patient Aligned Care Team (PACT) Outpatient Staff Registered Nurse (RN)
Major duties include, but are not limited to:
- Providing competent, evidence-based care to primary care patients and oversight of licensed vocational nurses/nursing assistants, as appropriate to the setting.
- Providing direct veteran care and care coordination, as it pertains, to veterans and their care within the primary care department.
- Managing assigned panel(s), utilizing registries, and ensuring appropriate evaluation and access is provided to patient panels.
- Providing health promotion, health education and coaching on wellness, disease prevention, and chronic care management.
- Assisting in directing the provision of nursing education, orientation, competencies and providing quality improvement and outcomes utilization.
- Demonstrating leadership in delivering and improving holistic, primary care through collaborative strategies with others.
- Contributing professional outpatient nursing perspective in discussions with the interdisciplinary team.
- Questioning PACT clinical practices for the purpose of providing evidence-based care.
- Assuming responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care.
- Administering medications and procedures per established policies and guidelines.
- This position potentially requires flexibility in schedule and assignments.
- All other duties as assigned.
