BBHE Care Coordinator-NCFIE

Salary: $49,700/year Position Summary Career coordinators facilitate family conferences and coordinate services for youth and young adults in or previously in foster care. They engage families, family managers, providers, and identified resources to promote child and family well‑being and to ensure compliance with federal, state, and local policies and regulations, including the Connecticut General Statutes governing program participation. The position is a 2‑year Federal grant funded role focused on coordinating care and supporting multi‑agency collaboration across the youth services department. Technical / Functional Expectations Family Team Conferences – Essential Function: Facilitate family conferences to achieve common outcomes coupled with family planning and specific client case plans, ensuring continuity of care and compliance with federal and state statutes and policies. Service & Treatment Delivery – Essential Function: Ensure the delivery and treatment of services, plan financial aspects, apply for resources, and prepare care plans substantiating funding requests. Utilization Management – Essential Function: Authorize and monitor services and resources, including funding requests, to meet sibling, family, and community needs. Relationship Maintenance – Essential Function: Foster and maintain ongoing relationships with family members, family managers, community partners, and identified resources, ensuring continuity of care and patient outcomes. Adherence to SOC Values & Principles – Essential Function: Adhere to one or more organizational SOC values and principles, demonstrating professional ethics, integrity, and reliability. Planning / Scheduling / Meeting Multiple Deadlines: Plan and schedule competing priorities, align resources, insert workforce capacities, allocate skill sets, and coordinate tasks to achieve the goals of the team. Other Duties as Required: Perform additional duties, including providing assistance to staff and client planning, supervising most daily quality fulfillment standards. Facilitation: Able to facilitate a leading advocate role within a foster care non‑profit organization, combining perspective and action to achieve desired systems. Development of Relationships: Able to develop and maintain partnerships with family members, staff, and community partners to strengthen engagement andvalues. Customer Service: Able to support clients and customers. Influence: Able to positively change opinions and actions. Planning: Provide strategic, approaches, resources, and data how determine; position: problem solving. Sensitivity: Sensitivity to cultural nuances, domestic privilege, systemic variances and understanding of community culture. Ethics: Adhering to values of agency, system of care foundation, and articulate the practice of internal standards. Experience Requirements High school diploma or GED (official transcripts required) AND four or more years of full‑time social work or human services experience. Associate Degree Requirement: Associate degree from an accredited college or 60+ college credits from an accredited college AND two years of full‑time social work or human services experience or two years of professional work experience. Associated examples: Guardian Ad Litem or similar child advocate role Family Support Worker Group Home Staff Teacher’s assistant/aide Childcare provider/staff Therapeutic Assistant Behavioral health technician Family intervention specialist Home health aide Nurse (LPN or RN) or similar profession Nursing facility assistance Emergency Medical Services (EMS) Other professional jobs that require an assessment of factors that can contribute to trauma or protective capacities with children and families. Bachelor’s Degree Requirement: Bachelor’s degree preferred. Related degrees include Criminology, Psychology, Sociology, Counseling, Special Education, Education, Human Development, Child Development, Family Development, Marriage and Family Therapy. 1–2 Years Related Experience Required. 2–5 Years Wrap‑Around Experience Required. Must obtain a Child Welfare Case Manager Certification within 12 months of completion of paid pre‑service training.
