PT - Physical Therapist

Outpatient Physical Therapist

This is a 13-week contract position.

Monday to Friday, 5x8 hours shift.

The Outpatient Physical Therapist will evaluate all assigned patients and develop, implement, and modify treatment plans according to the patients' needs, goals, and physician directives. This role is responsible for the delegation and direction of all discipline-specific therapy services for the assigned patients. The physical therapist will ensure the design of functional treatment programs that enable effective rehabilitation and return to community activities.

• Complete a comprehensive evaluation of assigned patients and develop individualized treatment plans.
• Document medical records in a clear, concise, accurate, and timely manner.
• Provide care under signed, valid physician orders and in compliance with insurance requirements.
• Participate in student programs, offering supervision and evaluation.
• Engage in professional development through continuing education and self-study.
• Adhere to the discipline-specific state practice act and maintain professional licensure.
• Understand and implement organizational performance improvement methodologies.

• Degree in Physical Therapy from an accredited school.
• 1+ year of outpatient clinical experience.
• Current CPR certification.
• Active licensure to practice as a Physical Therapist in the relevant state.
