Position Summary:
We are seeking a dedicated Psychiatric Nurse Practitioner (PMHNP) to provide comprehensive psychiatric evaluations, diagnosis, medication management, and patient education in an integrated behavioral health setting. The ideal candidate will treat a diverse patient population, develop individualized treatment plans, and support whole-person wellness. This role collaborates closely with therapists, case managers, and healthcare providers to coordinate high-quality patient care. Candidates must possess an active Illinois Nurse Practitioner license, with PMHNP certification preferred. Strong clinical, communication, and documentation skills are essential for success in this position.

Roles & Responsibilities:

• Perform rapid assessment, triage, and prioritize patient care based on acuity.
• Conduct comprehensive psychiatric evaluations, assessments, and diagnoses for patients across various age groups.
• Prescribe, monitor, and manage psychiatric medications while providing ongoing follow-up care.
• Educate patients on mental health conditions, treatment plans, medications, and wellness strategies.
• Deliver supportive counseling and behavioral health interventions to promote overall well-being.
• Collaborate with therapists, case managers, and healthcare providers to coordinate integrated patient care.
• Maintain accurate clinical documentation, participate in treatment planning, and refer patients to appropriate resources when needed.
