Job Summary Patient Care Technician (PCT) serves as a clinical support partner in Intermountain Home Health and Hospice, performing patient care duties under licensed professionals and sometimes as a Health Unit Coordinator. Responsibilities Prioritize and deliver basic patient care: toileting, bathing, changing linens and clothing, providing fresh water and ice, ambulating patients, administering comfort measures, and responding to patient and family requests. Document care findings and data promptly, and report changes in patient condition quickly. Perform telemetry duties as required and monitor patients’ needs. Transport medications from the delivery system to the RN and collaborate with the RN and interdisciplinary team for optimal care. As a Health Unit Coordinator, answer phones, transcribe physician orders, manage unit records, organize and maintain unit supplies. Implement delegated plans of care and perform duties within scope of practice. Perform specialized procedures with skill validation and in accordance with regulatory standards. Maintain a safe environment by following safety policies and responding appropriately to emergencies. Qualifications Current Certified Nursing Assistant (CNA) certification in the state of practice. Basic Life Support (BLS) certification for healthcare providers. Basic computer literacy and excellent written and verbal communication skills. Valid driver’s license, insurance, reliable transportation, and acceptable driving record. At least 18 years old. Ability to transfer and position patients independently and safely, and to lift heavy items as required. Ability to access the Internet or a phone line to transfer patient files twice a day. Physical capability to perform job duties (manual dexterity, lifting, bending, walking, standing). Preferred Qualifications Prior CNA experience or applicable healthcare experience.
