Salary: $57,242/Annually - Salary is Negotiable (Employees will receive 5% below the negotiated pay rate during their probationary status.)

Responsible To: Behavioral Health Supervisor

Summary: Provide consultation utilizing an integrated team-based model to adults and families to improve psychosocial functioning. This position will be based in the Diabetes Care Program within the Oneida Community Health Center. This position will expand into other areas of the Health Center.


Minimum Qualifications: Applicants please clearly state how you meet these qualifications on the application/resume.

• Master's degree in social work from an accredited program or related disciplines appropriate to the position.
• Must be licensed in the State of Wisconsin as a Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), Licensed Clinical Psychologist.
• Two (2) years of experience providing direct counseling services.
• Must be billable and reimbursable for commercial insurance, and medical assistance.

• Must provide a copy of diploma/degree, license, or certification upon employment.
• Employees are required to have proof of seasonal influenza vaccination; medical or religious exemption may be requested upon hire.
• May complete a pre-employment assessment prior to hire.
